How to use the form?

  1. Download the Form: Click below to download the return form as a PDF file.
  2. Fill in the Form: Fill in the required information on the return form. Give the exact reason for the return and/or exchange, e.g. if you want to exchange the product for a different size, colour or model. Make sure you enter the correct return and contact details.
  3. Prepare the Product: Prepare the product to be returned in its original box, with all tags, and without any signs of use.
  4. Prepare the Parcel: Make sure that the returned product is properly packed. The parcel should also contain the proof of purchase and a completed returns form.
  5. Shipping Charges: Please be advised that the costs associated with return shipping and re-shipping with the replaced product are to be paid by the customer.
  6. Deliver the Return: Deliver the parcel of the returned product to the address: LEDAPOL Dariusz Krawczyk, ul. Cegielniana 23, 42-100 Lubliniec.
